We believe that both the youth and the Parish beneﬁt from positive recognition at St. Bernard and through the Diocese of Lafayette. There may be occasions for media coverage including your student throughout the year, through shared video or pictures. We ask permission to release this type of communication. This could include, but not limited to: Public Media: Newspaper, TV, Catholic Moment, any other public media. Church Media: Bulletin Boards, Parish Videos, Newsletters, Projection Screens, any other church media. Web Based Media: Parish Web Site, Other related ministry and religious web sites, social media.

LIABILITY RELEASE
I understand that the Diocese of Lafayette-in-IND, St Bernard Pastorate employees, any volunteers, cannot assume any liability for people attending St. Bernard religious education programs or youth ministry programs and events. I waive, release and discharge the Diocese of Lafayette-in-IND, St. Bernard Pastorate employees, and any volunteers involved from any and all claims of liability from my/my child’s participation in these events.
